The Transgender District is back and excited to host our name and gender change support clinic in person on September 17th, 2025, from 3 p.m. to 6 p.m. at our headquarters located at 1067 Market Street, Suite #2001 San Francisco, CA 94103.

We understand that certain factors have created extra barriers when it comes to changing your name and gender on government-issued IDs. This intake form allows us to create a seamless experience for you and determine your best path toward updating your forms of identification.
